ASSESSMENT

START SERVICES!

​​​​​​STEP 1
Fill out waitlist form & insurance verification form, which can be found here. Once insurance is verified, we will contact you regarding our availability and your status.  Then, we will request your child's diagnostic evaluation report. 


STEP 2

When we have availability for your requested time(s) for services, an intake appointment will be scheduled with one of our board certified behavior analysts to discuss your family's treatment needs.  Information from this visit will be used to set up an assessment authorization from your insurance provider.


STEP 3

Once we receive authorization from your insurance, we will set up an appointment for an assessment with one of our board certified behavior analysts.  Assessment findings will be used to develop a treatment plan which will be sent to insurance for treatment authorization. 

STEP 4

When we receive authorization for treatment from your insurance, your child can start receiving services with an individualized treatment team! We will contact you regarding your start date and answer any questions before your first day.
