FILL OUT INSURANCE VERIFICATION & ENROLLMENT FORMS (Located in the "For Parents" tab)


STEP TWO

ONCE INSURANCE IS VERIFIED, WE WILL EMAIL YOU REGARDING AVAILABILITY OR WAIT LIST STATUS.  WHEN SPACE IS AVAILABLE, WE WILL REQUEST YOUR CHILD'S DIAGNOSTIC EVALUATION REPORT.  


STEP THREE

WHEN WE HAVE AVAILABILITY FOR YOUR REQUESTED TIMES, AN INTAKE APPOINTMENT WILL BE SCHEDULED WITH A BOARD CERTIFIED BEHAVIOR ANALYST TO DISCUSS YOUR FAMILY'S TREATMENT NEEDS.  INFORMATION FROM THIS VISIT WILL BE USED TO SET UP AN ASSESSMENT AUTHORIZATION FROM YOUR INSURANCE PROVIDER.


STEP FOUR

ONCE WE RECEIVE AUTHORIZATION FROM YOUR INSURANCE... WE WILL SET UP AN ASSESSMENT APPOINTMENT WITH A BOARD CERTIFIED BEHAVIOR ANALYST TO EVALUATE YOUR CHILD.  THIS INFORMATION WILL BE USED TO DEVELOP A TREATMENT PLAN WHICH WILL BE SENT TO INSURANCE FOR AN AUTHORIZATION FOR TREATMENT.

STEP FIVE

ONCE WE RECEIVE AUTHORIZATION FOR TREATMENT FROM YOUR INSURANCE, WE WILL SET UP YOUR CHILD'S FIRST DAY AND PROVIDE MORE DETAILS ON WHAT TO BRING AND WHAT TO EXPECT.  THIS IS WHEN YOUR CHILD BEGINS TO RECEIVE AMAZING ABA SERVICES WITH A MOST AWESOME THERAPEUTIC BEHAVIORAL TREATMENT TEAM!
